What you will do:

  • Lead a multi-disciplinary team to effectively deliver a nurse-led service aimed at preventing unnecessary hospital admissions.
  • Administer medication rounds and dispense controlled substances with precision and care.
  • Attend emergency response situations, conduct thorough patient assessments, and perform reception screenings.
  • Provide exceptional, evidence-based care, ensuring that care remains personalised and informed by patient feedback.
  • Deliver non-judgemental care that genuinely impacts patient outcomes positively.
  • Inspire excellence by establishing and maintaining strong connections with partners and stakeholders, providing a trusted and respected service.

Qualifications required:

  • Registered Nurse with specialism in Adult care and a current NMC professional qualification.
  • Proven experience in line management, executing audits, and maintaining governance standards.
  • Holding a mentoring assessor qualification and prescribing credentials is highly desirable.
  • Demonstrated evidence of continuous professional development.
  • Comprehensive understanding of the intricacies of multi-disciplinary and multi-agency working.
  • Ideal candidates will have practical experience within hospital, community, or prison environments, although this is not essential.
