What You'll Do<ul><li>Deliver high-quality nursing care, health education, and screening services.</li><li>Manage long-term conditions such as diabetes, respiratory disease, and cardiovascular health.</li><li>Carry out immunisations, wound care, travel health advice, and minor procedures.</li><li>Support QOF and Enhanced Services requirements with accurate coding and record keeping.</li><li>Work autonomously and collaboratively within a multidisciplinary team.</li></ul>What We're Looking For<ul><li>NMC Registered Nurse qualification.</li><li>Previous experience in general practice and managing long-term conditions.</li><li>Excellent communication skills and a patient-centred approach.</li><li>Ability to work independently and as part of a team.</li><li>Ideally, previous experience in primary care.</li></ul>Who are we? They are looking for Practice Nurses to provide support over the winter period mainly with QOF. You will be introduced to the team upon arrival and shown round, all access and logins sorted for you for when you arrive. The surgery have a Practice Nurse who has been there for over 20 years, which can show how much they look after their staff!Duties inclide:- Manage long-term conditions (e.g. diabetes, COPD, hypertension) including reviews and monitoring. - Administer immunisations and vaccinations for adults and children. - Carry out cervical screening and family planning (pill checks, contraceptive advice). - Conduct respiratory assessments and reviews. - Maintain accurate clinical records and contribute to audits/quality improvement. - Work collaboratively with GPs, HCAs, and the wider practice team.
