About the Role As an ACP, you will:<ul><li>Provide advanced clinical care through telephone triage, remote consultations, and face-to-face assessments for patients with undifferentiated and urgent health needs, including paediatrics, elderly care, and mental health.</li><li>Diagnose and manage complex cases using advanced assessment, examination, and clinical reasoning skills.</li><li>Prescribe and manage medications as an independent prescriber, ensuring safe and effective treatment plans.</li><li>Order and interpret investigations (laboratory and radiology) and make evidence-based decisions on care management.</li><li>Coordinate care across services, acting as a referral agent and care coordinator to ensure seamless patient journeys.</li><li>Lead on clinical quality and safety, promoting best practice, supporting audits, and contributing to service improvement initiatives.</li><li>Mentor and support colleagues, providing clinical supervision, coaching, and contributing to the development of junior staff and students.</li><li>Safeguard vulnerable individuals, applying policies and legislation in child protection, mental health, and safeguarding adults.</li><li>Work flexibly, including evenings, weekends, and bank holidays, and travel to different IC24 sites as required.</li></ul>What We Expect<ul><li>Professional registration (NMC, HCPC, GPhC) with post-qualification experience.</li><li>Advanced assessment and minor illness qualifications (Level 6 or 7).</li><li>Independent prescribing qualification (Essential).</li><li>Proven experience in autonomous practice and clinical leadership.</li><li>Strong communication skills, able to manage complex and sensitive situations.</li><li>Ability to prioritise, problem-solve, and make decisions under pressure.</li><li>Commitment to continuous professional development and reflective practice.</li></ul>Desirable:<ul><li>Experience in chronic disease management.</li><li>Teaching and assessing qualification.</li><li>Remote consultation experience.</li><li>Evidence of service improvement and innovation.</li></ul>Who are we?
