Detailed job description and main responsibilities The SMART Service is nurse led, therefore the ability to work autonomously as well as effectively within a team is essential. The work will include visiting patient in the community therefore it is essential that you have a full UK driving license and a car. You will triage, assess, plan and implement the care of patients who are discharged home under the care of the SMART service in the community, review and give care to SMART patients attending SDEC/ Hot clinic for review, administer IV treatments and conduct virtual visits using our remote patient monitoring pathways. The Staff Nurse must also be able to care for OPAT (Outpatient parenteral antibiotic therapy) and post-operative patients. As a SMART nurse teaching, auditing, infection control practices and undergoing research are activities undertaken within the role. Job overview We are seeking a qualified nurse to come and work in the SWK MHT+ team. If you are interested in older adults as well as in working age adults and keen to develop your expertise in initial assessments and therapeutic interventions, this is the role for you!
