This role, with the option for Full or Part Time hours, offers a 3 month Block Booking and operating within the hours of 09:00 - 17:00. What you will do:

Undertake holistic assessment, planning, implementation, and evaluation of patient care within the Discharge to Assess (D2A) pathway.
Deliver evidence-based nursing interventions in patients’ homes, focusing on supporting rehabilitation, recovery, and independence.
Facilitate early supported discharge and admission avoidance through proactive care management.
Work collaboratively within a multidisciplinary team, liaising with health, social care, and voluntary sector partners to coordinate care.

Qualifications:

Registered Nurse (Adult) with an NMC Active PIN
Relevant pre-registration nursing degree or diploma

Registration & Compliance:

Current registration with the Nursing and Midwifery Council (NMC)
Knowledge of NHS clinical governance frameworks and CQC standards
Understanding of the Mental Capacity Act and safeguarding principles
Commitment to Continuing Professional Development (CPD) and revalidation requirements

Experience:

Post-registration experience in a community, acute or integrated care setting
Experience supporting patients with complex health needs in the community
Exposure to discharge planning or admission avoidance services is desirable
Experience working collaboratively across health and social care systems
