What you will do:
- Conduct physical and mental health assessments for individuals accessing the services, ensuring proper referrals for further care when necessary.
- Lead assessments for alcohol service users and assist in determining safe detoxification plans.
- Support the titration process for opiate users, ensuring optimal dosing and clear communication with team members and community pharmacists.
- Provide education, health promotion, and screenings for blood-borne viruses, alongside administering Hepatitis B vaccinations as needed.
- Undertake community detoxification for alcohol and opiates, either at the service locations or in the service users' homes.
- Contribute to the Naloxone Strategy by training individuals in its use and distributing it to service users.
- Offer wound care assessments and basic wound treatments, with referrals to other services as appropriate.
- Engage with service users through Integrated Test and Evaluation Plans (ITEP) to enhance recovery planning and treatment engagement.
- Maintain and communicate up-to-date knowledge of clinical practices and interventions, attending relevant meetings and training under the guidance of the Sister/Charge Nurse.
- Drive the delivery of high-quality care services, constantly reviewing practices to align with clinical guidelines and protocols.
- Provide clinical leadership by supporting and coaching staff, ensuring their continued professional development in harm reduction and treatment approaches.
