What you will do:

• Conduct comprehensive health assessments and deliver clinical interventions in community and street-based settings.

• Manage and deliver care to your own caseload, ensuring efficient and effective episodes of care.

• Work both independently and collaboratively within multidisciplinary outreach teams.

• Support individuals experiencing homelessness and complex physical and mental health needs.

• Build strong partnerships with other outreach services and maintain clear communication pathways.

Key Experience Required:

• Proven experience delivering street outreach services successfully.
