Practice Nurse Prescriber

We are looking for a Practice Nurse Prescriber to onboard to a new service within the Blackpool community.

The Target Demand Management Service aims to provide clinical and non-clinical interventions that meet the needs of individuals identified in Blackpool through a mixed case-finding approach including the Core20Plus5 cohorts and with the following Long-Term Condition - cardiovascular disease (CVD) and/or COPD. You will need to have a prescribing qualification and undertake Long Term Conditions especially respiratory - inc spiro / feno.

Schedule: Mondays, Tuesdays, Wednesdays Thursdays - 08.30-17.00

Start Date: 13th April – 3 month block booking, potential extension.

System: Emis

Pay: Starting at £30/hr

Day to Day Duties to include, but not exhaustive:

• To provide proactive reviews and support for patients with CVD and / or COPD in line with clinical competence and local pathways
• Undertake holistic health assessments, identifying physical, mental, and social needs
• Deliver health prevention and health promotion interventions including lifestyle advice (alcohol, smoking, diet, and physical activity), weight management and wellbeing support, Opportunistic screening, and vaccination advice
• Identify unmet needs and escalating health concerns, taking appropriate actions
• Undertake Long Term Conditions especially respiratory - including spiro / FeNO.
• Deliver care within the mobile clinical unit adapting consultations to a community-based environment
• Support people who present to the mobile unit by providing assessment and advise where appropriate, signposting and referring to the most suitable service when out the scope of practice and offering reassurance, health information, and ongoing support
• Promote self-management and empower patients to engage with health services
• Accurately record all activity, assessments and outcomes on EMIS using appropriate clinical coding and template

Requirements:

• NMC Registered
• V300 Independent Prescriber Qualification
• Trained in respiratory, LTC, COPD
• Able to commute to Blackpool locality
