Practice Nurse - Primary Care
The Role
Primary care nursing is about more than treatment - it's about prevention, education, long-term condition management and helping patients achieve better health outcomes.
At IC24, we're looking for a Practice Nurse who is passionate about delivering high-quality patient care and enjoys working as part of a multidisciplinary team. This is an opportunity to use your clinical expertise across a broad range of nursing services while supporting patients at every stage of their healthcare journey.
Working in accordance with the NMC Code of Conduct, you will provide assessment, treatment, screening, health education and clinical advice to patients. You will work within practice protocols, policies and patient group directions, delivering care independently and alongside GPs, nursing colleagues and the wider healthcare team.
This role is ideal for an experienced Practice Nurse with expertise in chronic disease management, cervical screening, wound care, health promotion and preventative healthcare.
What you'll focus on
→ Delivering exceptional patient care
You'll provide high-quality nursing assessment, treatment and health education services, ensuring care is delivered safely, effectively and in line with best practice guidelines.
→ Managing long-term conditions
You'll support patients with chronic diseases including asthma, diabetes and cardiovascular conditions, undertaking reviews, monitoring treatment effectiveness, identifying abnormalities and developing individualised management plans.
→ Promoting health and wellbeing
You'll undertake health screening, NHS Health Checks, risk assessments and health promotion activities, helping patients identify and manage potential health concerns through education and preventative care.
→ Providing specialist nursing interventions
You'll deliver cervical screening, wound care, immunisations, travel health services, ear care, phlebotomy, ECGs, blood pressure monitoring, blood glucose monitoring and smoking cessation reviews.
→ Managing clinical risk
You'll identify significant abnormalities, recognise safeguarding concerns and ensure appropriate referrals are made for vulnerable adults, children and patients requiring additional support.
→ Supporting quality and service delivery
You'll ensure investigations required for QOF and Enhanced Services are completed and accurately coded, contributing to high-quality clinical outcomes and service performance.
→ Leading and supporting colleagues
You'll provide support and guidance to Healthcare Assistants and General Practice Assistants, contributing to supervision, workforce development and continuous improvement across the nursing team.

You're likely someone who:
• Holds a current NMC Nursing qualification and registration
• Has previous nursing experience, ideally within a general practice setting
• Has experience in chronic disease management, including asthma and diabetes
• Is confident undertaking cervical screening, wound care and health assessments
• Demonstrates excellent communication and interpersonal skills
• Understands the importance of confidentiality and information governance
• Has a patient-centred, holistic approach to care
• Is confident, adaptable and committed to continuous improvement
