Salary: £48,000 Pro Rata + Mileage An exciting opportunity has arisen for an experienced Complex Care Nurse to join a growing specialist care provider delivering high-quality complex care to adults and children within the community. Based in Devon, you will take responsibility for a current caseload of 4 complex care packages, with the opportunity to support the continued development of services across the region. The caseload will predominantly involve paediatric clients. Key Responsibilities
- Manage a caseload of complex care packages, ensuring high standards of clinical support
- Complete clinical assessments, care plans, risk assessments, audits, and care package reviews
- Investigate and respond appropriately to clinical incidents
- Provide clinical advice and support to care teams, clients, and their families
- Support the recruitment, development, and competency of complex care teams
- Deliver and support clinical training and education programmes
- Work closely with multidisciplinary teams across hospital and community settings
- Attend commissioner and MDT meetings where required
- Participate in regular case management meetings
- Monitor the quality and effectiveness of care packages
- Maintain up-to-date professional knowledge and clinical practice
- Participate in a rotational on-call arrangement for out-of-hours clinical support
- Active NMC Registration
- Nursing degree
- Minimum of 1 year's post-registration experience with tracheostomy and/or ventilator care
- Previous experience within community complex care
- Teaching, mentoring, or clinical education experience
- Full UK driving licence and access to a vehicle
- Excellent communication and interpersonal skills
- Ability to work autonomously and manage a clinical caseload
- Proven leadership or management experience is desirable
- £48,000 pro rata
- Mileage and expenses covered
