Job Purpose:

To provide high-quality, evidence-based nursing care to patients within their homes and community settings. As a core member of the multidisciplinary team, you'll focus on promoting independence, wellbeing, and preventing unnecessary hospital admissions - ensuring continuity and excellence in care across Sussex.

What You Will Do:

  • Conduct holistic nursing assessments and create personalised care plans.

  • Deliver a wide range of nursing interventions, including wound management, medication administration, palliative care, and long-term condition management.

  • Monitor patient progress and escalate clinical concerns appropriately.

  • Support seamless hospital discharges and ensure effective communication between healthcare teams.

  • Promote health education and empower patients to manage their conditions independently.

  • Maintain accurate clinical documentation in accordance with NMC and Trust guidelines.
