What you will do
• Mental Health Assessments: Conduct initial and comprehensive assessments of health and social care needs, both independently and alongside colleagues.
• Care Coordination: Manage care coordination responsibilities for a designated caseload of service users, promoting recovery and social inclusion.
• Care Programme Approach (CPA): Act as a Care Coordinator under the CPA framework, ensuring high-quality care planning and delivery.
• Multi-Disciplinary Working: Work closely with psychiatrists, social workers, psychologists, occupational therapists, and other professionals to provide integrated care.
• Mental Health Support: Deliver person-centred interventions and support individuals experiencing a range of mental health conditions.
• Primary Care Liaison: Provide mental health liaison and link-working support with primary care services and partner agencies.
• Clinical Leadership: Supervise and support junior staff members, students, and trainees, contributing to workforce development.
