What you will do:

Be an integral part of the Community Mental Health Team, delivering secondary care to older people with organic and functional mental health challenges.
Engage in comprehensive assessments, and devise short, medium, and long-term care plans, providing essential support and education to service users with a diverse range of psychiatric conditions.
Employ your expertise to guide those who do not require secondary mental health intervention toward alternative services.
Develop and implement treatment plans for individuals facing both organic and functional mental health issues.
Provide compassionate face-to-face and telephone support, addressing the unique needs of each service user sensitively.
Collaborate with senior team members to mentor junior staff and maintain a harmonious team dynamic through supervision and shared caseload management.
