What you will do:

Conduct comprehensive mental health assessments, including risk assessments, within a dynamic multi-disciplinary assessment team. Collaborate closely with the Consultant Psychiatrist to ensure a holistic team approach.
Efficiently refer patients to appropriate clinicians within the service or to external services/agencies when necessary to ensure seamless patient care.
Deliver a rapid, responsive service to evaluate individuals' needs for access to mental health services, ensuring timely intervention.
Foster effective communication between GPs, IAPT, and secondary mental health services to maintain the highest standards of patient care.
Provide mentorship and support to colleagues, contributing to a culture of learning and development within the team.
