Key Responsibilities
- Provide specialist palliative care to patients with complex clinical and psychosocial needs
- Manage and coordinate a defined patient caseload, ensuring high-quality, individualized care
- Offer expert advice on pain and symptom control, including physical, emotional, and spiritual support
- Collaborate with multidisciplinary teams to plan, implement, and evaluate patient care
- Support patients and families in discussing and planning end-of-life care preferences
- Deliver education, mentoring, and training to healthcare staff to enhance palliative care practices
- Participate in clinical audits, quality improvement initiatives, and service development
- Act as a key link between hospital services and community-based care providers
Requirements & Qualifications
- Registered Nurse qualification with valid professional registration
- Bachelor's degree in Nursing (or equivalent)
- Postgraduate qualification in a relevant specialty (or willingness to complete)
- Significant post-registration clinical experience
- Proven experience in supportive and palliative care
- Strong communication, leadership, and decision-making skills
- Experience in teaching, mentoring, and knowledge sharing
- Ability to manage complex patient needs and challenging clinical situations
- Experience in audit, research, or quality improvement is an advantage
- Prescribing qualification (desirable but not essential)
