Where: Countywide across West Sussex (This is a fully on-site position. Remote or hybrid working arrangements are not available for this role.)

Hours: Full time 37.5 hours

Contract Type: Permanent

Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.

Responsibilities

Purpose of the role

The Advanced Nurse Practitioner (ANP) – Homelessness Outreach will provide advanced, autonomous clinical care to individuals experiencing homelessness across West Sussex, with a particular focus on addressing health issues related to substance misuse and alcohol dependency. Working flexibly between service hubs and outreach settings, the post holder will deliver comprehensive health assessments, urgent primary care including wound care/immunisations/BBV treatment pathways, and ongoing management of substance‑ and alcohol‑related health conditions alongside co‑existing physical and mental health needs. The role requires independent clinical decision‑making, non‑medical prescribing, and close collaboration with multi‑agency partners to respond effectively to the complex and often interrelated health and social needs associated with homelessness. Reducing health inequalities and improving access to timely, appropriate healthcare for this underserved population is a central priority of the role.
