Day Webster are currently recruiting a Band 6 CPN to work with the community pathway team in Canterbury.

Job description:
The assessment of both urgent and routine mental health presentations.
Risk formulation, dialog care planning.
Offering therapeutic intervention.
System liaison (GP, social care, third sector partners.
Clinic working, monitoring a person's physical health and administering treatment for their long term mental health conditions.
Care planning, risk formulation, safety planning and patient outcome measures.

Essential:

Physical health, depot and clozapine competencies.

4. NMC regsitration
