Where You’ll Be Working

Join the Diabetes Service within Tamworth Community Health Service, a small, dedicated team of Registered Nurses and a Dietitian based at Tamworth Hospital. You'll deliver inpatient and outpatient diabetes care for all ages, including Type 1, Type 2 diabetes and gestational diabetes, within a multidisciplinary framework collaborating with internal and external providers for holistic patient support.

About the Role

As a Clinical Nurse Consultant (CNC) – Diabetes Service, you'll provide advanced, complex diabetes management using expert clinical skills and contemporary nursing knowledge, reporting to the Nursing Unit Manager (NUM), Chronic and Complex Care Service. Lead quality audits, service reviews, district-wide diabetes initiatives and research; act as the primary contact for non-clinical enquiries; and offer specialist outreach, mentoring and guidance to clinicians in sites like Gunnedah, Narrabri, Moree and Quirindi.

About You

You're an autonomous leader with expert diabetes knowledge, strong patient and stakeholder engagement, and the ability to deliver effective education and skill development to colleagues.

You bring:

  • Registered Nurse with current Authority to Practice with AHPRA who has at least 5 years full-time equivalent post registration experience, with at least 3 years full-time equivalent experience in this specialty field
  • Eligibility to drive in NSW and the ability and willingness to drive for work purposes
  • Graduate Certificate in Diabetes Management (essential)
  • Credentialled Diabetes Educator (desirable)
  • High degree of autonomy in clinical practice
  • Expert clinical skills and knowledge in diabetes management
  • Strong leadership ability and stakeholder engagement
  • Capacity to provide education, mentoring and guidance to teams

You'll receive robust support from the current CNC, team members, the diabetes stream and HNELHD Nursing and Midwifery Directorate for a smooth transition into this impactful role.
