Trainee Critical Care Outreach Nurse Practitioner
Job overview
The Shrewsbury and Telford Hospital NHS Trust is recruiting a Trainee Critical Care Outreach Practitioner (CCOP) to join the Critical Care Outreach Team (CCOT) during the expansion to a 24/7 Trust-wide service. This role is ideal for a compassionate, motivated individual committed to improving outcomes for acutely deteriorating patients.
The CCOT provides early recognition, assessment, and management of deteriorating patients across the Trust. As a Trainee CCOP, you will deliver patient-centred care, assess and manage critically ill patients, provide autonomous specialist advice, and work collaboratively with the multidisciplinary team to support escalation decisions and facilitate timely admission to critical care.
The role includes follow-up assessments of patients discharged from critical care, participation in the cardiac arrest team, and providing clinical leadership and support in the day-to-day operation of the outreach service.
The successful candidate will undertake a structured academic and clinical training programme aligned with the National Outreach Forum CCOP Framework, including an Enhanced Clinical Practitioner (ECP) Apprenticeship and non-medical prescribing qualification. Successful completion of all academic and clinical competencies within the required timeframe will lead to progression to a substantive Band 7 Critical Care Outreach Nurse Practitioner post.
Informal visits and discussions about the role, service development, and the Trust are welcomed.

Our Poppy’s Promise Commitment
We believe that compassionate care sits at the heart of everything we do. Every member of our team — clinical or non-clinical — plays a vital role in ensuring that patients, families, and colleagues are treated with dignity, empathy, and respect at all times.
We expect all staff to demonstrate compassion not only through their words, but through their actions: by listening attentively, communicating clearly and kindly, and recognising individual needs, feelings, and circumstances of those in our care.
Effective communication is central to safe and high-quality healthcare. We value colleagues who take the time to understand others, explain information in a meaningful way, and respond to concerns with patience and understanding.
By joining our team, you are committing to uphold these values — ensuring that every patient feels heard, respected, and valued, and that compassion is embedded in every interaction, every day.
