An exciting opportunity has arisen for an experienced and motivated Registered Nurse to join the East Lancashire District Nursing Service as a Senior Nurse within the Clinical Triage Hub. This permanent Band 6 role offers an excellent opportunity for a skilled District Nurse to further develop their clinical leadership, decision-making and triage expertise within a supportive and team.

The Clinical Triage Hub is central to ensuring patients receive timely, safe and effective care across the community. As a Senior Nurse, you will play a role in assessing and prioritising referrals, supporting the coordination of community nursing services. Using your District Nursing experience, you will make autonomous clinical decisions that help ensure patients receive the right care, at the right time.

Working closely with the Clinical Team Leader and multidisciplinary partners, you will contribute to service development. You will also mentor students and new staff, act as a role model to promote excellence in patient care.

We are seeking an enthusiastic and compassionate nurse with excellent communication, organisational and clinical assessment skills who is committed to delivering high-quality, patient-centred care. The role requires flexibility to work across a seven-day service, including rotational day and evening shifts, weekends and Bank Holidays. As a Senior Nurse within the Clinical Triage Hub, you will play a key role in delivering safe, effective and responsive community care. You will undertake clinical triage and assessment of referrals, ensuring patients receive the right care at the right time whilst supporting clinical decision-making across the District Nursing Service.

Working closely with the Clinical Team Leader and wider multidisciplinary teams, you will provide professional leadership, support workforce planning and staff allocation, and contribute to service development. You will also mentor students and new team members, promoting a culture of learning, excellence and continuous improvement.

We are looking for an experienced and enthusiastic nurse with strong clinical assessment, communication and leadership skills, who is passionate about delivering high-quality patient care and supporting colleagues to achieve the best outcomes for patients.

  • Please note that the salary advertised is for full time hours. If this post is less than 37.5 hours per week, the salary will be pro-rata. For posts on Agenda for Change pay-scales, new entrants to the NHS will normally commence on the first pay point of the relevant band.

Disclosure and Barring Scheme (DBS)

The healthcare sector is exempted from the Rehabilitation of Offenders Act 1974.

In line with other NHS organisations in the North West Region, the Trust is now passing the charge for undertaking a DBS check on to candidates in the event they are successfully appointed into the post for which they have applied. Candidates can choose whether to pay this over 1-3 months as a deduction from the monthly salary. However, if you are applying for a post as a BANK worker, the payment must be made in full at the time of employment checks. By applying for this vacancy you are agreeing to this undertaking in the event you are successfully appointed.

The cost of an Enhanced Disclosure will be £55.38 and for a Standard Disclosure will be £27.38.
