Role Purpose

The Qualified Social Worker within the Adults CHC Team plays a vital role in assessing, coordinating, and supporting individuals through the Continuing Healthcare process. The role ensures that adults with complex health and social care needs receive appropriate, person-centred support that promotes dignity, wellbeing, and independence.

What You Will Do

  • Undertake strengths-based assessments in line with the Care Act 2014, clearly identifying needs and outcomes.

  • Participate in Continuing Healthcare assessments alongside health professionals, including completion of Decision Support Tools (DSTs).

  • Support individuals and families through the CHC process, providing clear explanations of eligibility decisions and outcomes.

  • Develop, review, and monitor person-centred care and support plans to ensure they remain proportionate and outcome-focused.

  • Work collaboratively with NHS Integrated Care Boards, clinicians, care providers, and other professionals across the CHC pathway.

  • Manage a caseload of adults with complex and high-level needs, coordinating reviews and multi-disciplinary meetings.
