The Role
Primary care nursing is where clinical expertise, prevention and continuity of care come together.

From long-term condition management and preventative health to immunisations and wound care, you'll use your broad clinical skills to deliver outstanding patient-centred care as part of a supportive multidisciplinary team.
At IC24, we're looking for an experienced Practice Nurse to join our Primary Care team at Herstmonceux Integrative Health Centre. This is an exciting opportunity for a nurse with a passion for long-term condition management, particularly diabetes and respiratory care, to play a key role in improving outcomes for patients within a thriving general practice.

You'll work within an integrated healthcare system designed to improve access, reduce pressure on secondary care services and ensure patients receive the right care, in the right place, at the right time.
What you'll focus on
→ Managing long-term conditions
You'll play a leading role in supporting patients living with diabetes, asthma and COPD, delivering evidence-based reviews, monitoring treatment plans and helping patients take control of their health through education and self-management.
→ Delivering preventative healthcare
You'll provide NHS Health Checks, cervical screening, smoking cessation support and wider health promotion initiatives, helping patients improve their health and wellbeing through proactive care.
→ Immunisations and travel health
You'll administer adult and childhood immunisations, provide travel health advice and vaccinations, and ensure vaccines are managed safely in line with national guidance and Patient Group Directions.
→ Providing clinical nursing care
You'll deliver a wide range of nursing interventions including wound care, phlebotomy, ECGs, blood pressure monitoring, spirometry, cervical cytology and other treatment room services while maintaining excellent infection prevention standards.
→ Supporting holistic patient care
You'll assess patients' physical and emotional wellbeing, identifying when additional support or referral is required and working collaboratively to ensure patients receive coordinated, person-centred care.
→ Contributing to quality improvement
You'll maintain accurate clinical records, support audits and QOF achievement, contribute to service development and continually develop your own knowledge through evidence-based practice and professional development.
→ Working collaboratively
You'll work closely with GPs, nurses, healthcare assistants and the wider multidisciplinary team to deliver safe, effective care and continually improve the services we provide to our local community.

You're likely someone who:
• Is a Registered Nurse with current NMC registration
• Has previous experience working within General Practice or Primary Care
• Has strong experience managing long-term conditions, particularly diabetes and respiratory disease
• Is confident undertaking chronic disease reviews and delivering patient education
• Has experience providing immunisations, cervical screening and health promotion services
• Demonstrates excellent communication and interpersonal skills
• Is committed to evidence-based practice, continuous professional development and delivering exceptional patient care
