The Role
Practice nursing is where clinical expertise, continuity of care and prevention come together.
It's where you support patients to stay well, manage long-term conditions, and access the right care at the right time - often building long-term relationships that make a real difference to people's health and wellbeing.
At IC24, we're looking for an experienced Practice Nurse to join our Primary Care team across Allied Medical Practice and Ship Street Surgery in Brighton. This is a varied and rewarding role where you'll use your full range of nursing skills to deliver high-quality, patient-centred care within a supportive multidisciplinary team.
You'll work across a busy primary care environment, delivering everything from chronic disease management and immunisations to health screening and minor treatments. You'll also play an important role in prevention, early intervention and health promotion across the local population.

What you'll focus on
→ Delivering high-quality nursing care
You'll provide assessment, treatment, screening, health education services and advice to patients, working in accordance with the NMC Code of Conduct and practice protocols and guidelines.
→ Managing long-term conditions
You'll diagnose, monitor and support the development of individual management plans for patients with conditions such as diabetes, respiratory disease and cardiovascular disease, working in partnership with patients and other healthcare professionals.
→ Prevention, screening and health promotion
You'll undertake NHS Health Checks, cervical screening, immunisations, travel health advice, smoking cessation support and wider preventative health interventions.
→ Clinical procedures and treatment delivery
You'll carry out wound care, ear care, phlebotomy, ECGs, blood pressure monitoring and other routine clinical procedures safely and in line with practice policy.
→ Supporting quality and clinical governance
You'll ensure accurate coding and recording of patient data to support QOF and Enhanced Services and contribute to high standards of clinical governance and best practice.
→ Working within a multidisciplinary team
You'll work both independently and in participation with GPs, HCAs, General Practice Assistants and wider clinical colleagues to deliver safe, effective and coordinated patient care.
→ Risk assessment and safeguarding
You'll recognise safeguarding concerns and wider risk issues (including domestic abuse, child protection and vulnerable adults), ensuring appropriate escalation in line with policy.

You're likely someone who:
• Is a Registered Nurse with current NMC registration
• Has experience working in general practice or primary care
• Has experience managing long-term conditions
• Demonstrates strong clinical judgement and safe decision-making
• Communicates clearly and effectively with patients and colleagues
• Is confident working independently and as part of a team
• Has strong organisational skills and attention to detail in clinical documentation
• Is committed to delivering compassionate, patient-centred care
