Practice Nurse (LTC) - Torquay

Salary - Negotiable DOE

A coastal practice based in Torquay has a vacancy for a Practice Nurse to join their supportive team. They would be looking for a full time nurse, who specialises in long term conditions.

The successful candidate will have worked as a Practice Nurse in general practice previously, as well as being able to see patients for long term conditions (COPD, Asthma, Diabetes and CHD).

The surgery are interested in hearing from forward thinking Practice Nurses with ideas of how to implement new processes, as well as being part of a new development for the surgery.

The practice is based in Torquay, with a patient base of 8500 with a mixed patient demographic. The CQC rating for the surgery is 'Good' in all areas. The surgery are happy to support CPD, they have onsite parking available to staff.

In addition to the full scope of Practice Nurse duties, the post holder will: Undertake home-based and care-home-based QOF reviews for patients with long-term conditions
