An exciting opportunity has become available for a Band 6 Nurse Practitioner to join the newly commissioned Unscheduled Care Coordination Hub (UCCH) at Wirral University Teaching Hospital.
This exciting role offers the chance to lead and influence the delivery of a new, innovative model of urgent care coordination that is transforming the way patients access community-based services.
The UCCH has been developed to centralise and standardise the management of Urgent Community Response referrals, including Category 3, 4 and 5 referrals received from Northwest Ambulance Service (NWAS). Through effective triage, coordination and clinical decision-making, the Hub supports patients to receive timely care within their own communities, avoiding unnecessary hospital conveyance wherever possible.
As a key clinical leader within the Hub, you will work closely with Urgent Community Response teams across Wirral, West Cheshire, East Cheshire and Warrington, as well as colleagues within NWAS and wider system partners. You will provide expert clinical oversight, manage complex referrals, utilise Directory of Services, and ensure patients are connected to the most appropriate care pathway quickly and effectively.

Advert
Prevent unnecessary hospital admissions by developing an effective and collaborative interface between relevant agencies and services, ensuring the most appropriate and efficient care pathways for patients.
Manage patients with unplanned care needs, including hospital admission where clinically appropriate. This includes prioritising telephone assessments for all patients contacting the Clinical Assessment Service via NWAS.
Provide appropriate advice and support to patients and carers in managing urgent care needs, and where necessary, arrange timely referrals to a wide range of suitable services.
Ensure that all patients are referred to the most appropriate service or agency to meet their individual care needs.

For substantive employees, the DBS costs are recovered through salary deductions following commencement in post in 3 equal instalments.
