A well-established and forward-thinking Primary Care Network (PCN) in Cheshunt is looking to recruit an experienced full time Advanced Nurse Practitioner (ANP) to join its dedicated clinical team. This is a fantastic opportunity for an experienced ANP to join a supportive and collaborative PCN that is committed to delivering high-quality patient care, professional development, and excellent services to the local community.

About the Role:

Working autonomously as part of the wider primary care team, you will provide high-quality clinical assessment, diagnosis, treatment, and management of patients. During this role, you will be working collaboratively with GPs, Practice Nurses, HCAs, Pharmacists, and other members of the multidisciplinary team.

The role will include:

• Autonomous patient consultations and clinical assessments
• Diagnosing and managing a broad range of acute and chronic conditions
• Developing and reviewing appropriate treatment plans
• Prescribing within your scope of practice
• Providing clinical advice and supporting patients with ongoing health needs
• Supporting the delivery and development of PCN services
• Maintaining high standards of clinical governance and patient safety

Job Requirements:

• Current experience working as an Advanced Nurse Practitioner within primary care
• Independent prescribing qualification
• Strong autonomous assessment, diagnosis, and treatment skills
• Experience managing both acute and chronic conditions
• Excellent clinical decision-making and communication skills
• A proactive, patient-centred, and team-focused approach
• Commitment to maintaining high standards of clinical care and governance
