What you will do

• Work within a multi-disciplinary team to deliver a nurse-led service and prevent hospital admissions.
• Conduct medication rounds, dispensing controlled medication, and attending emergency response situations.
• Assess patients and undertake reception screening to ensure immediate healthcare needs are met.
• Utilise your expertise to deliver nurse-led clinics, supported by a skilled multi-disciplinary team.
• Contribute to our nursing strategy and vision by providing outstanding evidence-based care and seeking feedback to personalise care.
• Deliver non-judgemental care that makes a genuine impact, guided by our bespoke Health in Justice competency framework.
• Inspire excellence by building strong ties with partners and stakeholders, ensuring a trustworthy and safe service.

Requirements

To excel in this role, you must have a legal right to work in the UK, and due to the sensitive nature of this position, employment offers will be subject to additional vetting and security checks.

• Registered Nurse - Adult with current professional qualification (NMC).
• Demonstrated evidence of Continued Professional Development.
• A deep understanding of the complexities inherent in multi-disciplinary and multi-agency working.
• An empathetic and non-judgmental approach to care.
• Exceptional communication skills.
• Familiarity with clinical governance, health and safety, QOF, EBP, and national standards.
• Strong knowledge of healthcare policies and best practices.
