What you will do:

• Conduct comprehensive assessments of individuals struggling with potential dementia, providing timely diagnoses and treatment planning.
• Develop personalised care plans for patients, coordinating with caregivers and family members to ensure holistic support.
• Offer post-diagnostic support and implement problem-solving strategies for patients and their carers.
• Liaise with allied professionals and refer patients to pertinent support services for continued management and support.
• Work closely with the Alzheimer's Society to facilitate ongoing research participation and community resource connection for patients.

Requirements:

• Degree or equivalent qualification in Mental Health Nursing.
• Proven experience as a Mental Health Nurse is essential.
• Current NMC Registration.
• Ability to work 9-5 on Fridays consistently, with flexibility for two other days among Monday, Wednesday, Tuesday, or Thursday.
