Enhanced Practitioner End of Life / Palliative Care
Job overview
The enhanced clinical level of this role will require you to be able to undertake enhanced level comprehensive assessment including history taking, physical examination of the patient and a robust systems review. You should be able to assess and manage frailty and complex chronic disease, identify deterioration and implement immediate strategies to reduce the risk of acute admission. Furthermore you will need to be able to recognise the early symptoms of disease exacerbation, acute illness and injuries, based on an understanding of chronic diseases, the disease process, current evidence and practice standards.
You should be competent to recognise that a person is deteriorating and advanced care planning might be appropriate. You will be expected to recognise that the person is coming into the last days of life and that dying phased care is appropriate.
You should be competent to undertake a level 3 medication review and the non medical prescribing qualification would be beneficial or alternatively a commitment to develop your skills by undertaking the module in due course.
*Please note that this role will be based in the North & East of the county in Cornwall and will involve travel across the locality, it will also involve working nightshifts to cover the service out of hours*

· Deliver advanced clinical assessment, diagnosis and management for adults with complex, frailty, long-term condition, palliative and end-of-life care needs.
· Develop, implement and evaluate person-centred care plans to support patients to remain safely within their own homes and communities.
· Work collaboratively within a multidisciplinary team and with partner organisations to provide integrated care and prevent avoidable hospital admissions.
· Provide expert clinical decision-making, identifying and responding appropriately to changes in patient condition and escalating concerns when required.
· Undertake Level 3 medication reviews and contribute to medicines optimisation within scope of practice.
· Support advance care planning and the delivery of high-quality palliative and end-of-life care.
· Maintain accurate clinical records and ensure safe, effective and evidence-based care.
· Provide clinical leadership, advice and support to colleagues, contributing to service development, quality improvement and best practice initiatives.
· Participate in training, supervision and continuing professional development activities.
· Contribute to the delivery of the service across North and East Cornwall, including participation in the out-of-hours rota and night shift working.
