The Anticoagulation & DVT Nurse Specialist is responsible for delivering a safe, effective, and patient-centred nurse-led Anticoagulation and Deep Vein Thrombosis (DVT) Service across West Suffolk NHS Foundation Trust.
Working autonomously and as part of the multidisciplinary team, the post holder assesses referrals, monitors and manages anticoagulation therapy, provides specialist clinical advice, and supports patients throughout their treatment pathway.
The role involves assessing patients for suspected DVT, initiating and monitoring anticoagulation treatment in accordance with evidence-based practice, Trust policies, clinical guidelines, and Patient Group Directions (PGDs).
The post holder provides expert counselling and education to patients, families, and carers, including training and ensures patients receive safe and effective care throughout their treatment journey.
The post holder is responsible for the assessment and management of ambulatory patients with suspected DVT, including clinical evaluation, diagnostic investigation, treatment initiation, referral, and follow-up. They work closely with primary and secondary care teams to coordinate care, facilitate timely interventions, and manage urgent situations such as anticoagulation reversal when required.

Key responsibilities include providing specialist advice to healthcare professionals, maintaining accurate electronic patient records using anticoagulation management systems, supporting clinical decision-making, and ensuring effective communication across multidisciplinary teams.
The role contributes to high standards of patient safety, continuity of care, service efficiency, and clinical governance while promoting best practice in anticoagulation and thrombosis management.
