· Use advanced clinical judgement to deliver evidence-based interventions, monitor response to treatment and adapt plans promptly when presentation or risk changes.
· Undertake crisis triage and contribute to gatekeeping assessments for psychiatric admission, ensuring that viable alternatives are considered and that decisions are clearly documented, defensible and based on the least restrictive safe option.
· Make and communicate sound clinical decisions in complex or rapidly changing situations, escalating significant concerns appropriately and supporting coordinated responses with emergency and statutory services.
· Administer, monitor and promote the safe use of medicines, and undertake relevant physical health assessment and monitoring within professional scope, competence and Trust policy.
· Coordinate care across the multidisciplinary team, inpatient wards, community services, primary care, social care, emergency services and voluntary-sector partners to support admission avoidance, continuity of care and timely discharge.
