What you will do:

• Undertake comprehensive assessments for patients eligible for Continuing Healthcare funding
• Develop and implement person-centred care plans in collaboration with multidisciplinary teams
• Communicate effectively with patients, families, and healthcare professionals
• Maintain accurate documentation in line with NHS and local authority guidelines
• Advocate for patients, ensuring their needs and preferences are central to care delivery

Requirements:

• Registered Nurse with a valid NMC registration
• Experience within Continuing Healthcare (CHC) assessments
• Strong communication and assessment skills
• Ability to work independently and within a multidisciplinary team
