What you will do:

• Conduct CHC assessments, including Checklists, Decision Support Tools (DSTs), and Fast Track assessments.

• Coordinate and participate in multidisciplinary team (MDT) meetings, ensuring robust and evidence-based eligibility decisions.

• Develop and review care packages and support plans, ensuring they meet individuals’ assessed health needs.

• Monitor and review existing CHC cases, ensuring continued eligibility and appropriate use of resources.

Person Specification:

Qualifications:

• Registered Nurse (RMN, RGN, or RNLD).

• Evidence of continued professional development.

Registration & Compliance:

• Active registration with the Nursing and Midwifery Council (NMC PIN).

• Strong knowledge of the National Framework for NHS Continuing Healthcare, Care Act 2014, and Mental Capacity Act 2005.

• Enhanced DBS clearance.
