What you will do:

• Work independently and collaboratively within an interdisciplinary team to assess, plan, and implement bespoke care programs.

• Conduct complex assessments, drawing on your clinical expertise to tailor effective interventions.

• Navigate diverse work environments, adapting to the unique demands each setting presents.

• Communicate complex and sensitive information effectively with patients and their families using both verbal and non-verbal tools.

• Continuously evaluate patient progress and adapt treatment plans as necessary, involving patients in their care decisions and ensuring informed consent.

• Manage and prioritize your caseload, supervising and delegating tasks to junior team members and support staff.

• Develop comprehensive discharge plans, in collaboration with a multidisciplinary team, to ensure seamless transitions for patients.

• Contribute to clinical education, offering support and guidance to unregistered staff, patients, carers, and students alike.

• Strategize partnership engagements with relevant agencies to enhance patient outcomes.

• Maintain personal competen
