We are recruiting nurses at our Rockhaven and Heatherstones services.

Our clients at Rockhaven are adults with learning disabilities, complex health needs and physical disabilities. Rockhaven is a 7 bedroom bungalow in Horsforth, Leeds with close links to the community.

Heatherstones is an 8 bedded bungalow in Queensgate, Halifax. Supporting clients with physical disabilities, learning disabilities and nursing needs.

Multidisciplinary teams are actively involved in ensuring that our clients are supported with best clinical health practices including social care that is evidence based.

The teams are close knit, and we all work together to make the working environment fun, educational for staff and continuously focussed on the clients and their needs. You may pick up shifts at more than one service.

Your duties will include:

  • Supporting the management team to ensure all clients are supported to have the best person-centred care and ensure their loved ones are actively involved in their care. You and the team will be responsible for providing the highest standards of care to ensure that their daily activities and health care needs are effectively met to enable them live a life without limits.
  • Supporting the manager in ensuring that all staff employed within your service are recruited, developed, supported and managed in an appropriate and professional manner.
  • To support the manager in ensuring that the clinical needs of clients are appropriately assessed and that plans of care and support are developed and implemented which effectively meet with stated needs with the overall objective of maintaining the wellbeing of clients
  • Managing client medications in line with best practice ensuring the ordering, storage, administration and review is in line with policy. Ensuring risks are identified, mitigated and documented to safeguard the safety of the clients we support.
  • Responsible, with your manager, for addressing the training and clinical skills development needs of support staff and ensuring that they are reviewed against St. Anne’s service objectives and policies.
  • Responsible, with your manager, for addressing your own personal development and training needs through PREP and a professional portfolio, based on service objectives.

Detailed job description and main responsibilities

  • Responsible for supporting the manager in maintaining the quality of the service provided within your defined area.
  • Responsible for supporting your manager in ensuring that you and staff within your service comply with St. Anne’s values and beliefs, policies and procedures. In addition, that any legal or contractual obligations are fully met.
  • Responsible for supporting your manager in ensuring that your service(s) have access to, and uses, the best current thinking and practice in the delivery of those services.
  • Responsibility for contributing to the successful future development of St. Anne’s by highlighting opportunities within the broad corporate aims of the organisation. This will include contributions to policy and procedures and to service development.
  • To monitor clinical practice and standards of care, and the implementation of policies/practice guidelines and legislation appropriate to the service and to take corrective measures when required to do so.
  • Carry out all relevant clinical tasks required for clients support like medications, PEG feed, Catheter care, health and wellbeing monitoring. You will work well with Multidisciplinary professionals and support the coordination of care.
  • You will poses high communication skills and will use your IT skills and maintain high standards of documentation using our various electronic record systems. You will be responsible for ensuring your mandatory and role specific training and registration is kept in date.
