  • Weekdays - £25.00 per hour (Umbrella/LTD)

Key Responsibilities

  • Deliver specialist diabetes management and treatment plans in community and clinic settings.

  • Support patients and families with education and guidance on lifestyle changes and self-management strategies.

  • Collaborate effectively with GPs, dietitians, podiatrists, and other healthcare professionals to coordinate care.

  • Conduct comprehensive assessments and tailor care plans to meet individual patient needs.

  • Monitor patient outcomes, adjusting care interventions and treatment as required.

  • Promote best practice and contribute to continuous service improvement.

Essential Requirements

  • Registered Nurse (Adult) with active NMC registration.

  • Proven experience in diabetes care within community or primary care settings.

  • Strong patient education and communication skills.

  • Ability to work autonomously while being part of a collaborative multidisciplinary team.

  • Excellent clinical judgment and commitment to patient-centred care.
