We have an exciting opportunity for a Band 6 Community Psychiatric Nurse (CPN) to join the Primary Care Team in Weston-super-Mare. This full-time locum role offers a competitive pay rate of up to £26 per hour over an initial 3-month contract. What you will do:
- Carry out comprehensive mental health assessments, identifying the nature and severity of mental health conditions and differentiating between physical and psychological symptoms.
- Develop, implement, and review person-centred care plans that promote recovery, independence, and positive mental health outcomes.
- Work collaboratively with GPs, primary care professionals, social care teams, and other partner agencies to provide coordinated care.
- Deliver specialist mental health interventions while working autonomously and exercising sound clinical judgement.
- Undertake carers' assessments and provide support and education to families and carers where appropriate.
- Mentor, supervise, and assess students and junior staff, supporting their professional development.
- Apply current mental health legislation and relevant legal frameworks to ensure safe, effective, and compliant practice.
- Maintain accurate clinical documentation and utilise electronic systems to support efficient service delivery.
- Communicate effectively with patients, carers, colleagues, and external agencies, demonstrating excellent verbal and written communication skills.
