This full-time locum role is within the esteemed Older Adults Community Mental Health Team (CMHT), offering £26 per hour over three months. What you will do

• Conduct thorough memory assessments and provide specialised care for older adults with psychiatric conditions in the community.
• Develop and implement personalised care plans to manage patients’ mental health and wellbeing effectively.
• Work collaboratively with the CMHT to deliver integrated, tailored support for older adults.
• Travel within the Caterham area for home visits and coordination with other healthcare providers.
• Maintain meticulous records and documentation to monitor patient progress and outcomes.
