What you will do

• Manage a defined caseload under the Care Programme Approach (CPA), delivering comprehensive mental health assessments, risk assessments, and evidence-based interventions.

• Develop, implement, and review person-centred care plans in partnership with service users, carers, and partner agencies, promoting recovery and independence.

• Act as Care Coordinator, ensuring integrated care delivery across health and social care systems, including liaison with GPs, social services, and voluntary sector providers.

• Participate in MDT meetings, contributing to clinical decision-making, safeguarding processes, and service development initiatives.

Person Specification
Qualifications

• Registered Mental Health Nurse (RMN) with degree-level qualification or equivalent.

• Evidence of continued professional development (CPD).

• Mentorship or practice assessor qualification (desirable).

Registration & Compliance

• Current registration with the Nursing and Midwifery Council (NMC).

• Working knowledge of the Mental Health Act 1983 (amended 2007), Mental Capacity Act 2005, and Care Programme Approach.

• Understanding of NICE guidelines, safeguarding legislation, and NHS clinical governance frameworks.
