About the role

We are seeking a dynamic Band 6 Community Psychiatric Nurse (CPN) to join the Primary Care Mental Health Team within the Neighbourhood model in Bromley. This permanent, full-time role offers the opportunity to work at the forefront of integrated mental health care, supporting adults with moderate to complex mental health needs.

You will work closely with GP practices, Primary Care Networks (PCNs), and neighbourhood health and social care teams, focusing on early intervention, relapse prevention, and reducing unnecessary referrals to secondary mental health services. What you will do

Clinical practice

• Manage a defined caseload of adults presenting with common and severe mental health conditions within primary care.

• Undertake comprehensive biopsychosocial and mental health assessments, including risk and safeguarding assessments.

• Formulate, implement, and review person-centred care plans in collaboration with service users and carers.

• Deliver evidence-based nursing interventions, including brief therapeutic interventions, medication monitoring, relapse prevention, and crisis planning.

• Provide mental health expertise and consultation to GPs and primary care staff.
