What you will do
- Assessment: Conduct comprehensive mental health assessments for adults within the community.
- Care Coordination: Develop, implement, and review personalised care plans tailored to individual needs.
- Medication Management: Administer depot medications and support service users with medication compliance and education.
- Mental Health Support: Provide specialist care and interventions for adults experiencing mental health conditions including schizophrenia, affective disorders, eating disorders, and other complex presentations.
- Multi-Agency Working: Collaborate with GPs, social care teams, hospitals, care homes, and community services to ensure coordinated care.
- Recovery Planning: Support individuals in achieving greater independence through access to housing, employment, physical healthcare, and specialist services.
- Family Support: Work closely with families and carers, involving them appropriately in care planning and decision-making.
- Record Keeping: Maintain accurate, timely, and professional clinical records in accordance with service requirements.
