What you will do

  • Care Management: Lead and manage a caseload, delivering personalised care plans tailored to individual needs.
  • Assessment: Conduct comprehensive mental health and risk assessments to inform care delivery.
  • Care Coordination: Work closely with the multidisciplinary team to ensure integrated and effective support.
  • Physical Health Monitoring: Carry out physical health checks and oversee medication management.
  • Service User Support: Provide guidance, education, and relapse prevention strategies to service users and carers.
  • Audit & Improvement: Participate in audits, research, and risk assessments to support service improvement.

Qualifications / Requirements

  • Registration: Must be a Registered Mental Health Nurse (RMN) with valid NMC registration.
  • Experience: Previous experience within Community Mental Health Teams (CMHT) is essential.
  • Clinical Skills: Strong assessment, care planning, and risk management abilities.
  • Communication: Excellent interpersonal and teamwork skills.
  • Autonomy: Ability to manage a caseload independently within a community setting.
