What You Will Do:

Conduct Assessments: Undertake strengths-based assessments for new referrals within our community mental health teams. Regularly update risk assessments and create personalised, sustainable care plans.

Manage Cases: Oversee a caseload of service-users facing complex mental health issues, coordinating care under the Care Programme Approach (CPA).

Provide Support: Collaborate with service-users and carers to promote recovery and social inclusion, offering information and helping navigate available services.

Coordinate Interventions: Implement and monitor health and social care interventions tailored to individual needs, prioritising cultural sensitivity.

Engage Families: Work with families and carers to incorporate them into treatment plans where appropriate, offering psycho-education and support.

Collaborate with MDT: Engage with the multi-disciplinary team to deliver comprehensive, high-quality care. Participate in clinical meetings and offer support to colleagues as needed.

Develop Professionally: Stay up-to-date with best practices and complete all mandatory and ongoing training. Supervise and mentor junior staff and students on placements.
