What you will do:
- Be an integral member of the Brent Community Mental Health Team (CMHT), leading on primary care mental health liaison.
- Conduct initial and comprehensive assessments of health and social care needs.
- Co-ordinate and co-work on care responsibilities for an identified group of service users, with a commitment to recovery and social inclusion.
- Provide supervision to staff and students, fostering a collaborative and educational environment.
- Participate in MDT activities, including duty and clinic rota.
- Enhance communication strategies between Mental Health Services and General Practice Teams.
- Work within CNWL policies and professional frameworks, upholding organisational values.
- Engage with GP Practices to develop an understanding of the CMHT's role.
- Collaborate with the multi-disciplinary team on the mental health care pathway.
- Educate professionals and services on referral criteria and provide pre-referral advice.
- Conduct assessments, manage risks, and participate in clinical meetings.
- Implement person-centred care plans following best practice guidance.
- Facilitate mental health reviews and provide guidance on medication management.
- Strengthen ties with community and voluntary services for comprehensive support.
- Safeguard patients' rights and advocate for them in their care journey.
