What you will do:

• Conduct comprehensive assessments of patients and devise personalised care plans to meet their psychological and social needs.
• Act as a Care Coordinator, ensuring patients receive coordinated, timely, and effective interventions.
• Engage in Multi-Disciplinary Team (MDT) activities and participate in duty and clinic rotas.
• Offer supervision and support to junior staff and students within the team.
• Foster and maintain effective communication with General Practice Teams and other agencies to streamline care pathways.
• Participate in clinical meetings to discuss patient needs, share insights, and develop cohesive care strategies.
• Engage in health education, medication management, risk assessments, and the formulation of risk management plans.
• Collaborate with community and voluntary organisations to offer patients additional support and therapeutic options.
