What you will do:

  • Deliver a comprehensive mental health service, engaging in activities and interventions that support individuals' well-being.
  • Facilitate the delivery of Personalised Care and Support Planning, ensuring that each plan is human-rights focused, collaborative, and trauma-informed.
  • Engage in meaningful, strength-based conversations that help co-create advance plans for safety and well-being.
  • Record risk incidents and work collaboratively to create and update Safety Assessments and Plans.
  • Provide personalised interventions for individuals experiencing mental health challenges, both as an independent practitioner and within a Multi-Disciplinary Team.
  • Plan, deliver, and evaluate nursing interventions in line with recovery principles and secondary mental health service roles.

Report to: Team Manager, who will guide and support your journey, ensuring your contributions align with our shared goals.
