What You Will Do:

  • Conduct comprehensive mental health and risk assessments for service users.

  • Serve as a Care Coordinator under the Care Programme Approach (CPA), crafting and reviewing recovery-focused care plans with users and carers.

  • Deliver evidence-based nursing interventions, psychosocial support, and monitor medication compliance.

  • Assist in relapse prevention and crisis planning to foster a secure recovery environment.

  • Appraise and manage risks associated with self-harm, suicide, and safeguarding concerns.

  • Collaborate regularly with a multidisciplinary team, including psychiatrists, psychologists, social workers, and support staff.

  • Participate in team meetings, contributing to case reviews and discussions.

  • Support effective medication management, monitoring effects and administering depot medication when necessary.

  • Maintain thorough and legally compliant clinical records, engaging in ongoing professional development and adherence to the NMC Code of Conduct.
