What you will do:

• Undertake comprehensive nursing assessments and develop individualised care plans tailored to each patient.
• Manage a varied community patient caseload independently, fostering leadership and time-management skills.
• Deliver compassionate nursing care directly in patients' homes, offering personalised support where it is needed most.
• Monitor patient progress, routinely review care plans, and escalate concerns efficiently to maintain optimal health outcomes.

Requirements:

• Valid NMC registration with a commitment to maintaining high professional standards.
• Previous experience in community or district nursing.
• Proven ability to manage patient caseloads and work autonomously.
• Excellent communication and clinical assessment skills.
• Full UK driving licence and reliable access to your own vehicle.
