Job overview
An exciting opportunity has arisen for an experienced Nurse to work as an Associate Specialist Palliative Care Nurse (AfC Band 6) based in our friendly, supportive teams that cover North Norfolk, South Norfolk and Central Norfolk. Due to career progression we have part time and a full time vacancies in the team. The successful candidates will have a working base in line with the Place team they are working with.
The Community Specialist Palliative Care Nursing teams are based at locations across Norwich, North, South and West Norfolk providing expert clinical palliative and end of life care for patients in their own homes and care facilities.
The successful post holder will work across 3 duties in their role as a specialist Palliative Care Nurse; Triage, community visiting and on a palliative care advice line; a telephone advice service for professionals as well as patients and their family or carers.
The teams work as part of and closely with the wider Specialist Palliative Care Service, including a consultant-led specialist inpatient unit, a Psychological Therapies team and a Day Therapies service.

To deliver evidence-based clinical care for patients with life shortening illness' who have complex specialist palliative care needs, from the point of diagnosis onwards under the supervision of the community Specialist Palliative Care Nurses.
To manage their own supervised clinical caseload, supporting patients and their families within their homes and in outpatient settings. The post-holder will link closely with community nursing teams, urgent care teams, Supported Care, primary care, and the acute palliative care teams . The primary aim is to ensure continuity of care through all healthcare settings.
To undertake shifts as part of a clinical team operating the palliative care advice line service for professionals and patients/carers, providing clinical support and advice to any service users accessing this service.
To contribute to and be involved with audit, education/teaching and research.

Detailed job description and main responsibilities
- To provide holistic specialist palliative care nursing assessment of patients and carers with highly complex needs, identifying priorities of care, implementing strategies in order to give this care and evaluation to ensure the maintenance of clinical excellence.
- To manage their own supervised clinical caseload, supporting patients and their families within their homes and outpatient settings.
- To engage with the development of nursing practice in line with changing and expanding
clinical practice guidelines. - To have good knowledge of and understanding of specialist palliative care.
- To ensure that nursing care is based on sound and current research and initiate/participate
and promote nursing research and evidence-based practice. - To be competent in assessment for and recognising emergencies within palliative care.
- To attend and participate in multi-disciplinary team, Gold Standards Framework and
Specialist Palliative Care team/PLACE team meetings to contribute to safe, effective clinical
care, help to promote new ideas and methods for the delivery of care and assist in their
implementation through liaison and cooperation with colleagues and other professionals. - To attend clinical supervision.
- Participate in education, competency assessments and training.
- In accordance with Nursing & Midwifery Council scope of professional practice continue to
develop and expand your nursing role. - To contribute to a positive learning environment for all grades of nursing staff and students.
- To undertake/participate in any audits resulting in any learning/action planning.
