Salary: £23.48 - £32.49 per hour (Band 8a)
Hours: Full-time - Monday to Friday (7.5-hour shifts)
Contract: Fixed-term until 31st March 2026

We're working with a healthcare provider in Kirklees who is seeking an experienced and confident Advanced Clinical Practitioner to join their Community Virtual Ward / Hospital at Home team. This is an exciting opportunity to work within an innovative service delivering acute-level care in patients' homes, supporting admission avoidance and early discharge.

The service operates across Huddersfield, Dewsbury, Cleckheaton, and Mirfield, and offers structured weekday working within a supportive multidisciplinary environment.

The benefits of working in this service:

• Band 8a role paid at £23.48-£32.49 per hour
• Fixed-term contract until 31st March 2026
• Monday to Friday working only
• 7.5-hour shifts (08:00-16:00 or 09:00-17:00)
• Mileage paid at 59p per mile (excluding commute to/from closest base)
• All equipment provided (excluding uniform)
• Opportunity to work within an expanding Virtual Ward / Hospital at Home model

The right candidate must have (essential):

• Advanced Clinical Practitioner qualification
• Experience managing Heart Failure and Frailty
• Proven community-based working experience
• Ability to work autonomously within a multidisciplinary team
• SystmOne experience
• Full UK driving licence and access to a vehicle for business use

The ideal candidate would have (desirable):

• Strong background in managing complex long-term conditions
• Experience supporting admission avoidance and early discharge pathways
• Confident clinical decision-making in a community setting
